ORGANIZATION OVERVIEW: 

Face to Face Health and Counseling is a non-profit organization dedicated to advancing economic and health equity for youth ages 11–24. We do this work through a comprehensive approach that includes housing, mental health, medical, youth justice, education & employment, and community services. We take a strengths-based and trauma-informed approach to our work and center the needs of young people impacted by racism and other forms of oppression and marginalization. Founded in 1972, Face to Face serves more than 3,000 youth annually. 

 JOB SUMMARY: 

The part-time Clinic Nurse Practitioner (NP) provides individualized healthcare services by diagnosing and treating health conditions typical of young people. The position works primarily with a team at our Arcade medical clinic. This position is for 22-30 hours and has the potential to include some administrative hours.

This role requires availability during our hours of operation which are Monday-Thursday 10:00 a.m-6:00 p.m. and Friday 10:00 a.m-4:00 p.m.

RESPONSIBILITIES:

  • Obtain patient history and perform assessment by observation, interview, and examination.
  • Order, perform, and interpret diagnostic studies.
  • Perform preventative health assessments, screening, immunizations, and patient care.
  • Provide direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
  • Perform clinical procedures approved by supervising provider or facility-based protocol.
  • Counsel and educate patients concerning preventative health, treatment options, and community resources.
  • Document in the electronic health record according to the Advanced Practice Registered Nurse scope or practice.
  • Preserve the patient’s control over decision making, assess the patient’s commitment to the jointly determined and mutually acceptable plan of care, and foster patient’s personal responsibility for health.
  • Participate as an integral member of the Clinic and SafeZone teams by promoting and demonstrating collaborative practice behaviors.
  • Engage in activities that promote professional growth and development to enhance processes that contribute to effective, efficient, and safe patient care.
  • Provides all services guided by the evidence-based principles for helping youth overcome homelessness.
  • Perform other duties as assigned.

 QUALIFICATIONS:

  • Master’s level degree and NP certification as required by your governing board.
  • Current Advanced Practice Registered Nurse (APRN) licensing in Minnesota required or must be obtained prior to hire date.
  • Experience in sexual and reproductive health with experience in implanted devise procedures (IUDs and implants) preferred.
  • Must be non-judgmental and able to build rapport with youth who have experienced trauma and have a passion for meeting patients where they are.
  • Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds.
  • Excellent computer skills and experience with electronic health records.
  • Prior experience with 340B program preferred, but not required.
  • Prior NextGen EHR experience preferred, but not required.

  • Requires standing and walking for extended periods of time.
  • Requires working under stress in emergency situations.
  • Exposure to communicable diseases, toxic substances, medicinal preparations and other conditions common to a clinic environment.
  • Requires manual dexterity and coordination sufficient to operate office and medical equipment.
  • Responsibilities may involve occasional lifting of equipment/supplies, usually less than 25 pounds.

PAY & BENEFITS: 

The pay rate for this position is between $57 - $59 an hour based on qualifications.

Face to Face offers full benefits at 30 hours a week of or more, which include health, dental, vision, short-term disability, long-term disability, life insurance, 403b account (3% employer match) as well as 23 days of PTO and 11 paid holidays. All benefits except health, dental, and vision are available at 20 hours per week or more.
